TBI Outpatient Program
The Traumatic Brain Injury (TBI) Outpatient Program uses an interdisciplinary approach toward providing unique and individualized treatment to patients with traumatic brain injury and their families, with a goal of improving their quality of life.
To Schedule an Appointment in the TBI Program:
New Patients
All new patients need a referral from their primary care doctor, an emergency department visit, or from another provider.
To provide the highest quality of care, all new patient referrals are reviewed by our TBI nurse to make sure that each patient is matched with the right type of brain injury provider. This process can take up to a week after a referral is received. You will be called when the process is complete.
- New Referrals should be faxed to our Referral Center at fax: 612-873-1635.
- We must have current patient contact information (address and phone) so that we can reach the patient, and the referral must include the date of the brain injury (or approximate date). If this information is not provided it may delay review of the referral and scheduling, and therefore delay treatment.
- For referral status questions, call the Referral Center at 612-873-6663.
Current Brain Injury/TBI Program Patients
Patients already in the program should call 612-873-9494 for scheduling needs, form assistance, or to talk with a TBI nurse. You may also use MyChart to cancel an appointment or send a message to your care team.

Who do we serve in the TBI Outpatient Program?
- Injury Type: Persons with any type of brain injury (concussion, mild, moderate, severe, non-traumatic, anoxic).
- Age: Adolescents aged 13+, and adults of any age.
- Time Since Injury: For mild brain injuries or concussions, the date of injury must be within the past 5 years. For more moderate or severe injuries, there are no limits on time since date of injury.
- Spinal Cord Injury: Persons with brain injury who also have spinal cord injuries are eligible for TBI clinic evaluation (either incomplete or complete up to level C7). We do not currently serve patients with recent complete spinal cord injuries beyond level C7.
- Limb Loss: Persons with brain injury who also have limb loss are eligible for TBI clinic evaluation.
- Medical Status: The TBI program generally serves medically stable persons who are living in the community either independently or with supervision or assistance. The program may also serve external patients who are in sub-acute or long-term rehabilitation settings for medication management or specialty care that is not availablein the subacute rehabilitation setting.
- Health Equity: The population served by this program includes individuals who are suffering from effects of a known or suspected brain injury regardless of race, national or ethnic origin, economic status, lifestyle, gender identity, sexual orientation, religious, spiritual, or philosophical beliefs, or language.

CARF Certification
The Commission on Accreditation of Rehabilitation Facilities (CARF), accredits Hennepin Healthcare’s Traumatic Brain Injury Outpatient Program as an Interdisciplinary Outpatient Medical Rehabilitation Program: Brain Injury Specialty Program for Adults and Adolescents. This accreditation demonstrates the TBI Outpatient Program’s commitment to outstanding care, quality improvement, safety, and patient satisfaction. The TBI Outpatient Program has been accredited since 2017. Accreditation lasts 3 years. We successfully re-certified in 2020, and will again in 2023-2024. At the last survey, the surveyors commented, “The interdisciplinary outpatient brain injury specialty program has several innovative initiatives due to a creative and talented team that includes neuro vision rehabilitation as well as comprehensive interdisciplinary staff with brain injury specialists in occupational therapy, speech-language pathology, neuropsychology, psychology, social work, nursing and physical therapy.”
